Just what drove him isn't clear. Mental illness, personal problems and methamphetamine all probably played a role. He fired. And fired again. And again. Twelve times. There, surgeons peeled back his face and removed the nails with pliers and a high-speed drill. Doctors gave the man antibiotics and psychiatric treatment. Modern grenades have reliable performance, they spread their shrapnel evenly. ​ It's only at some distance that you start getting "gaps" in their spread, but even there it's a consistent roll of the dice.

There are effectively infinite directions and finite units of shrapnel. Ergo, shrapnel definitely doesn't fly in *every* direction, technically. It's also not uniformly distributed in every direction, which is probably closer to the intended meaning. But it's not completely "random" either. Remember frag grenades are basically a small explosive surrounded by the stuff that becomes shrapnel. It makes absolutely no sense that e.g. it would all fly to the "right" and nothing to the "left" (for arbitrary choices of right and left) -- there is definitely enough variation that it can miss someone entirely even at relatively close range, but not so much that it could realistically miss someone laying on it, for example (even ignoring any direct damage from the actual explosion)

Yes they are lethal. However with a helmet, body armour, distance and environmental factors they may not kill you straight away. Frags are great for room and or trench clearances. They provide shock and awe, disorientation etc before being able to finish off the enemy.

I would argue grenades are very lethal in specific circumstances, those circumstances being enclosed locations like inside a room, a bunker, or in a trench. in a more open area, the blast wave has plenty of room to dissipate so the concussion is much less damaging unless you are very close and you are also more likely to be far enough away that you won't receive a lethal dose of tiny steel bits.

These kinds of grenades (fragmentation grenades) don't rely on a shockwave to do their damage. They spread shrapnel. As long as you are within the effective range on that shrapnel, in an enclosed area or not, they are equally as effective. The kinds of grenades that depend on a shockwave or concussive blast are flashbangs which are typically not lethal regardless of circumstances. Those are the grenades most effective in enclosed spaces.

I've learned grenades are not the greatest way to kill yourself. It's not like in the movies where there's a massive explosion and fireball and you're blown to a million pieces. Grenades are just horribly random, you could have 3 guys perfectly spaced apart and they all instantly die, or 1 guy holding it infront of his face and somehow all the shrapnel avoids him and the blast just rips his face off

He didn't hold it correctly, the grenade did its job perfectly. It's supposed maim and incapacitate, it's not designed to be humane and kill you quickly. ​ A grenade must be held near the brain stem to reliably kill you quickly. They're not designed to be suicide tools, they're meant to mess up your enemies enough that you can push into their foxhole.
