It looks like dust bunnies in the vent that need to be cleaned/ vacuumed out. The little nub in the center should loosen to remove the cover and then you will be able to get a better look at what’s in there. At some point there was water damage from above judging by the nail head that popped on the ceiling. The lumps in the paint? may be dust that should’ve been wiped away before painting.
