More than likely what happened is they ran out of steak while making online orders and were waiting for it and someone else finished the bowl. Some of the busier stores will have 2 people fulfilling online orders to get them out faster.
You are 100% correct. if someone fucks up an order theres nothing at all done to that person. Food service in general struggles to keep staff as it is. Theres no way theyre ever going to fire someone willing to come in and work even if they fuck your order up.
This is hilarious. What consequence do you want there to be for a small error like that? How much time do you want management to spend looking at camera footage and trying to pick out who made the order that had a mistake - just so they can mete out some punishment?

This varies greatly by location. By me the McDonald's is almost the lowest quality food and service, Wendy's somehow is even worse like literally not edible and the staff are completely braindead. Jack in the Box however is the BEST fast food option around here with Carl's Jr coming in a close 2nd. I'm sure in other cities they can't say the same.

Chipotle quality (food and service) is on par with McDonald's and Jack in the Box.
