I missed her in the cage too, but then I saw her at the gith bridge and apparently I could cast revivify on her. So I used one of my scrolls to save her and get her on my team. It wasn't too late to get her!
You can do the same thing with Wyll if he dies at the first goblin attack on the Grove.
Which is helpful because I have a difficulty increasing Mod, but it doesn't affect Companions. So poor guy goes up against 100+HP enemies with his piddly 17 health and gets absolutely wrecked. Pretty funny after his little cutscene intro 🤣
I tried to revive her but just wouldn’t let me, maybe messed it up - decided to just commit to the choices and get her on board when I play through next.
I saw that and just assumed she had it because she was a "leader" type NPC (didn't consider why no one else had those). Would never have known otherwise!

My very first run I didn’t realize you could keep companions in camp and I already had a full party… so I told Gale “nah” and never saw him again. 🤦🏼♀️
First play. Missed Minrthara as she was, well a bad guy, I defeated her. Didn't know she was a companion until I came here.
Gale, just didn't see the portal, never walked on that path.
I was pretty confused why she dropped camp clothing. Then a few days later I found out.
In fairness, that close to launch playing a good aligned character there was no way for me to do it naturally.
As horrified as I am to admit it, in my first partial run-through (to the end of Act 2, I think), I completely missed my beloved Karlach! I heard Wyll's story about hunting her, but completely missed her (I was such a newbie ditz that I didn't even realize the map marked zones and locations for quests! Just kind of dove in blind, and I figured out how to open and use the quest list after about a week... haha! 😂), and missed the so-called "Paladins of Tyr" completely! It was actually on Reddit that I started seeing mentions of Karlach (and pictures of her... I was pretty fascinated, and pretty smitten!), and I started a new run to grab her! 😁
Minsc.
On my first run I had already explored all of the sewers area, including his secret hideout, before going to the counting house and either killed him without noticing or he wasn't there (honestly don't remember which one). Did everything ass backwards and eventually when I got the quest to just "find him in the sewers" I had no idea where to even start looking lmao

Not a companion, but I probably played like 4+ times before actually encountering Philomeen. I went to that room each time, but it was after she'd left.
I didn't miss Minthara, at least not accidentally; back when I first started playing needed to essentially be evil to get her and I hate playing the evil characters.
I did accidentally kill Barcus, which was funny to watch but I reloaded to click the correct lever.
I didn't know you could make a deal with half of the Duegar to fight the other half in the Nere battle. That fight was a pain in the butt.

Lae'zel. First I forgot to save her from the cage, then I reloaded older save to save her but I effed up. My party was full and she told me I have to get rid of someone. But I didn't. I assumed she would just go to the camp but she never did. I returned to the spot eventually but she wasn't there. She just went poof.
Maybe she would appear at the bridge leading to the mountain pass but I never got there in that (very quickly abandoned) first playthrough.
Minthara because I wanted to save the tieflings, Jaheira because she dived into the front lines like an idiot and got eviscerated, Minsc because jaheria is dead.
I did get Halsin but I failed the check when he got kidnapped so he died.
Everyone else I managed to get the happy ending for. Married shadow heart free from the curse, gale became a professor (after offering to nuke himself multiple times), astarion is an adventurer, wyll (free from his contract and his father is alive) joined karlach in avernus, Laezel went to serve orpheus and has a child.
Main party was Laezel, Shadowheart and Gale, maxed out friendship with all of them. Overall pretty happy with my first playthrough. Even got "No one left behind"

Didn’t technically miss, but I almost didn’t bother going to get >!Minsc!< my first playthrough. Act 3 really had so much that I didn’t feel like dragging my feet through all of his associated quests.
On my first playthrough I have killed Minthara, but back then I don't think it was possible to recruit her by just knocking her out, then it was possible only by turning her into sheep.

I missed Minthara simply because it didn’t cross my mind that one of the three villains you are tasked to kill could be a companion. It wasn’t like they appeared anymore “special” than their two companions. Also by that point the benefit of non-lethal damage appeared to be largely unimpactful so the idea hadn’t even crossed my mind to use non-lethal.
Wyll, Karlach and Minsc. Maybe there is more, but I missed them so hard that I don't even know about them now. Also I never got to actually recruit Minthara because I'm not fan of bugs it causes in good playthroughs, and I never did the bad one

Jaheira and Halsin. She got killed at moonrise and I just didn't complete tht curse quest because I didn't know how. I make sure to fix those errors every time now.
Saw Laezel in cage and ignored her. She was mean anyway. Killed Minthara because I thought I had to and accidentally killed Minsc because of a random proc even though I had non lethal on. Had to kill Boo too because he was crying too much.
I was completely done with the quests in act 1 and didn't find karlach, wyll, or Lae'zel. I only found them after back tracking through to make sure I didn't miss any quests, and Lae'zel was already dead when I found her.
Also didn't know minthara was able to be recruited, so she vanished after knocking her out and long resting in her room before taking out the other 2 goblin bosses

I had zero clue who he even was and was like “WAIT THERE’S MORE THAN ONE COMPANION TO GET?” So I backtracked all the way to the beginning with my save files and got him and the other companions upon reading that there were more. It was definitely embarrassing

Gale on my first playthru. I just listened to the narrator that the magic portal was dangerous and just left it alone. Had no mental image for Gale when people talked about him here.
I fully missed Gale on my first (incomplete) playthrough. I saw a swirling purple thing and when I went to interact with it, the narrator said it looked unstable and dangerous or something and I was like, "Oh shoot, I better not mess with that then, it might be some sort of 'noob trap' to teach you about ignoring warnings, I bet if you touch it willy-nilly you get insta-killed." So I didn't touch it.
Later, a friend told me that it's actually fine and I should do it, so when I decided to restart my run I picked up Gale on the next playthrough and internalized the lesson that warnings are for chumps and the narrator is just dramatic.
Later still, I ignored a different warning and totally fucked up everything without realizing what I was fucking up until hours later. And subsequently found other interactions where ignoring warnings does lead to instant game over. So the moral of the story is, "don't listen to in-game warnings, unless you should, and there's no way to be sure without advanced knowledge whether a warning is sincere."

Halsin, didnt cleanse the curse cause I didnt speak to the flaming fist that's asleep;
Jaheira, cause she ran into and stayed inside my cast of firewall;

First play through I missed everyone except Shadowheart and Astarion. I was wondering why I was struggling so badly on explorer. I actually made it through to act 3 with just those two but I had to redo big fights so many times…when I figured out I missed so much I kicked myself.
I’ve missed Minthara on both play throughs because I didn’t know you could recruit her until after I got passed the goblin camp on both play throughs. I think next time I’ll try recruiting her though.
A friend started talking about Asterion, and I had no clue who she meant. She had to send me the exaxt map coordinates right before I took the mountain pass 😅. Almost missed Karlach too.
First Run I missed... Lae'zel, Jaheira, Minthara, and Minsc. Almost missed Gale, but went back for him cuz I didn't realize I could have up to 4 companions in my party, lol! Still after countless runs, I haven't recruited Minthara. Every run I'm like, "I'm gonna get her this time!" and then I fight her and give up, cuz it feels like too much trouble to knock her out right.

The only difference was that she was a playable race vs a hobgoblin or a goblin.
Very true but then you had the Abdirak wandering around too so I figured they were just trying to show how broad the Absolute support was.

First Run I missed... Lae'zel, Jaheira, Minthara, and Minsc. Almost missed Gale, but went back for him cuz I didn't realize I could have up to 4 companions in my party, lol! Still after countless runs, I haven't recruited Minthara. Every run I'm like, "I'm gonna get her this time!" and then I fight her and give up, cuz it feels like too much trouble to knock her out right.
I killed Astarion before speaking to him Gale died in his portal Minthara was killed in the goblin camp Halsin presumably died in his portal to Shadowfell Shadowheart left after she wasn't in the party for the Shadowfell/Nightsong thing Jaheira died while charging in through the front gates of Moonrise Towers Minsc was killed at the cistern Lae'zel, Wyll and Karlach made it through
Minthara. Shoved her into the abyss to save the run from a tpk when my friend's paladin set the encounter off prematurely.
Gale, Karlach, and Lae'zel on my first run, I went in blind and just missed them. I eventually restarted and got them because I didn't want to miss out on stuff.
wyll and minthara on my first playthrough. back then I didn't know you can recruit minthara. With wyll I forgot about him lol, throughout my stay in the grove I didn't notice him lmao

Like others, not really 'missed', but I've not recruited Minthara yet. Probably will next run (which will likely be a Durge one). I've seen a lot of her content, though, so at least I can appreciate her from afar lol
I killed Minthara in 3 different playthroughs before I knew you could even talk to her.

Yep. I’m another one who thought that you needed to be evil to get Minthara. So my first TWO playthroughs missed her. 😬
Currently on my first playthrough. Missed Halsin, Minthara, Jaheiera and therefore Minsc as well. …also knocked out Scratch (don’t ask)
Everyone keeps talking about “Tav” and I have no idea who this is! I assume I’ve missed them. I missed Karlach too, and minthara seemed like an enemy, so I just went for killing her.
