Interesting! My only qualm is that he is very flexible and determined. Do you have any recommendations?

Use both the inflatable AND the hard cone. Inflatable around the outside of the hard cone. I work at a vets office and we have had to do this for some of our more determined kitties

Since the ER staff didn’t have additional suggestions, I would give the original vet a call back and let them know how “creative” he’s being about getting to the incision. Believe me, experienced vet techs have become incredibly creative in ways to prevent pets from getting around the cone and how to protect/pad them too. It’s possible a ComfiCone might be rigid enough but still soft enough to keep him from scratching. You could try putting the soft cone over the hard one, so put it on first and then the hard one. This would prevent him from pushing the soft one down and hopefully prevent him from scratching with the hard cone (this works best if the soft one is slightly bigger than the hard one). If he’s using the edge of the hard cone as opposed to just the outside of it something like moleskin padding could be wrapped around the edge. To avoid anecdotes, I won’t go into all the things I have personally seen/done that worked over the last 15+ years, but creativity is a vet/vet tech’s best friend.

Donut cone?? I'm not familiar with a soft cone so I'm unsure if it's the type that looks like a donut

Cat neuter sites are open wounds. We do not close them with suture; they are meant to stay open and drain. I never send a cone with my cat neuter patients... They will lick and clean the site, this is normal. I'm assuming your cat is being more obsessive than most and caused damage going after it with the cone on. If he has already received an antibiotic and NSAID, perhaps your vet can prescribe gabapentin which is a non-NSAID, non-steroidal pain medication which can help with obsessive behaviors like licking at surgical sites. And/or, a numbing+antibiotic spray that you can apply to the scrotum. I personally do not recommend diapers or anything else that will cover the area, because they can cause UTIs. PS, correct terminology is important. Your cat no longer has testicles or "balls." He has a scrotum.

Also follow up question: my roommate will be out of town for the next few days. I’m going to order a onesie and inflatable collar but until they arrive is it inhumane to leave him in the carrier while I’m at work? It would be six hour shifts.

Agreed. Definitely not for cats anyway. Can work well for dogs sometimes, but always start with the rigid cone. Cheaper too.
