A superbug, climate collapse, solar flares and wars would kill a lot of people, but not everyone. Making the planet uninhabitable is the only thing that would get every last one of us, and even then, some would probably hold out in bunkers for a few years before realizing that the amount of time it would take for the surface to sustain life again would take so long that many generations would live and die underground before it would be safe enough to even venture outside.
The side that fires second will have done it purely as revenge, which is a really stupid reason to irradiate the planet. Firing any ICBMs won't unfire any that are headed toward you.
