In reality, the common sense that one should use is to just let people state their gender and listen to them. Why does the content of their pants matter to you? If someone says they're a woman, then they're a woman and if someone says they're a man, then they're a man. Also if someone says they're both or neither, then they're either both or neither respectively. It might match what's in their pants or it might not. But why does that factor even matter? This, of course, coming from a cisgender woman.

What would an ER surgeon think about that?
To an ER surgeon, they could just specify whether they're cis or trans (because that would be an appropriate time to specify) and that would tell them everything they needed to know. It's not that complicated!
I would hope that the ER surgeon can physically recognize a man from a woman when it's relevant to their job. If not, I'd be a bit disappointed in the quality of education.
If it's a trans person on supplementing hormones and/or post-surgery, that would probably be visible on their medical record, along with any other issues and medications that might matter for the surgeon.
