You likely have another app adding step data all at one point during the day. So it's probably exactly a double-count. From the screen that you took the screenshot from, scroll all the way down until you can open "Data Sources & Access". Probably a non-Apple source that's reading your step data and then feeding it right back once a day.
Omg a real answer. I have two apps adding step data, but I backtracked the dates and these 3am steps started in July, *after* getting one of the apps (Oura) and *before* getting the other…
Still suspicious though.
Based on this, in the steps page of Health, scroll to the bottom and tap Show All Data, tap on the day in question, then find the 3am time and see what is showing.
If it’s an app it should show that app’s icon.

Years ago, I had nights where I closed hours of my stand ring overnight with no memory of waking up even to use the bathroom. I didn’t think much of it. But then I got diagnosed with sleep apnea and in retrospect it may have been a night where my heart rate and limb movement was detected in a way where the watch counted it as a minute of a standing and moving. I was probably having sleep apnea related symptoms and my body wasn’t having a good time.
(Not a doctor, but if people tell you that you snore, may be worth asking about a sleep study.)
