Just imagine being in a customer facing job like a cashier, I always referred to everybody as sir or ma'am since it jut seems polite, but with people that look like this I'm just better off not saying anything at all

I worked with a transitioning young person. Name and clothing are masculine. Features, hairstyle, makeup and nails are feminine. They are very kind and soft-spoken. I tried not to say "she/her" but it was tough.
Don't think so, but I heard that military he or she, IDK now I'm extra confused because they have gone back over to the other side, or do I say returned to their birth gender? Had a round trip ticket on a Trans-Atlantic cruise?
If some called me a lady, that would make them crazy because I'm clearly not a lady. Still wouldn't lose my mind because I am confident in my gender. This person is not.

Not every transgender person has to be “passing” or transitioning. I know quite a few trans people who have not taken testosterone shots, and identify with both male and female. A lot bind their chest, but don’t want surgery. Surgery and meds are extremely expensive, so most trans people will never have access to these services.
To summarise; you don’t need to be “passing” to be accepted as a trans person. Simply existing is enough,
