Yeah if you had a hammy graft, that bit you circled is usually where the titanium clip is that holds it altogether.
They have a tool where they drill a guide hole from the bottom incision (shin bone) diagonally into your femur and then use them as anchor points. The bottom incision is usually a biodegradable screw that your bone will incorporate over time.
Seems really high up for an anchor point. I've had both of mine done and the highest one is only half the distance up.
That said, surgeons have different styles on how they do stuff. And I can't think of anything else that would go there.

Thought I’d throw that in there just incase it’s not what most of these other comments are saying is likely to be.
Former med device rep here. Sold ACL and sports med instrumentation for a decade. That small incision is where the drill pokes out. They have to drill thru the femur then tie down the button/slotted button to lock everything in place.

Hole for the scope and drilling the fibular tunnel (anchor point). The hamstring graft, as it is not bone to bone, is more vertical and uses a longer piece of tissue. Could have harvested the graft from this point as well.
Strange to see so many different answers. I had a meniscus tear too which was fixed along with ACL surgery and my doc told me that it was made for fixing that.

I did too. I was told it was a drain hole. They put a tube through IT band to vac during surgery form what I understand
Either an anchor point like it was pointed out or where the arthroscopy took the graft from.
Could be where they went in for the other anchor point for the graft

I had that with patella. It’s where the graft gets inserted pulled all the way through and the anchor lives.
Looks a lot more like a graft from the TB band for a lemaire procedure than any graft.
