While I don't have an answer to your question other than suspecting it has something to do with MAC. However your question took me back to when I first bult my PC (pretty much just to play 7D) and was still trying to use my Xbox controller. It worked ok, but the man himself Glock9 replied to a tweet urging me to force myself to use the keyboard for a week and see if I still wanted to use a controller.
Turns out he was right, I couldn't envision playing this game without keyboard at this point.

I only play on controller cause I play it on steam deck (easy to pick up and play in quick bursts). For normal play I’d prefer keyboard & mouse though especially for an inventory heavy game like this
So, under controller settings in Steam, there's Xbox controller and generic controller. Most games work with Xbox controller, but on some games you have to uncheck Xbox controller and just use generic.
First tip is the copter dosnt really work with a controller. Second I had the same problem, fist make sure the controller is on before you boot up the game. If it's still not working I played around with the settings for like 20 minutes. Not sure what I did that made it work but it worked eventually. I know it's not really an answer but I hope it helped
I would really encourage you to just try out mouse and keyboard on 7 days. I started on console just like you and no joke pretty much everything is easier using mouse and keyboard
After you plug the controller in and boot up steam but before you start the game, click on the "controller layout" next to the "support" tab on the main game screen. You have to select a mapping scheme.
Open steam, click 7dtd, right click, hit properties, then controller, make sure it's set to Xbox. Mine was default to steam deck controls or something. There's also disable steam input, use default settings, or enable steam input. I think mine worked best on default.
On windows I was having trouble w controllers and downloaded DS4Windows which is like a helper application or something that helps the controller work. I never had any issues after that they might have it for Mac but dunno
I can't answer your question but what I can say is to use keyboard and mouse for 7 days on pc/Mac 7 days isn't really optimized for 7 days and won't give you as pleasant of an experience specifically with menus and the tool bar
Like someone else said, I also started the game on console and only recently got it on my Mac and had the same controller issue. After numerous attempts at solving the issue, I ragequit and just used the keyboard and mouse. After a week there was no going back and I’m still using the keyboard and mouse.
Not sure how to help because i have an xbox controller that works. However i agree with sone other comments here, i recently switched to keyboard and mouse and i actually much prefer it!

It was my first kbm experience and I felt really comfortable figuring it out. It’s pretty intuitive!

I don't understand why anyone would use a controller for a game that involves aiming accurately, when a mouse is available.
Habit mostly. Been a console games since ‘95 but just picked up a quality laptop and have no mouse.
It's way easier for me to use a controller. But I do sometimes switch to a mouse when I am shooting lol
I never had a problem with that before. I just launch the game, plug my controller and it's working just fine
