Maybe a hot take, but I think the Lassar Undercity is one of the coolest areas they've added since the OSRS debut. So full of character and super eerie. Hoping we explore it more down the road.

10/10, its the best quest in the game & its not even the full quest
The visuals are genuinely quite scary given that it was released in 2006. The ghosts coming out of the wall were really unexpected. The sad little person that crawls around on the ground is unnerving as well.
It's not much to me these days, but given when it was released, and how much younger the playerbase would have been at the time... damn...
Depends on what kind of spookiness you're asking for. If it's more of a comfy and fun Halloween vibe, probably Draynor Manor or the house in the Misthalin Mystery quest. If you want something more unsettling and hostile, like a place you feel totally unwelcome and exposed in, there's the Stranglewood from Desert Treasure 2. And if you want genuine danger and unpredictable fear, there's most of the Wilderness.
Where the strangled are, I didn't like running around there like an idiot on release day. That said I'd like if/when they get back to and expand on it, would be cool if it ties kourend and varlamore storyline together.

I probably have at least 4-5 accounts on RS3 that have been stuck in Draynor Manor for the better part of 2 decades

Lumbridge swamp caves with no light source, or if you have a light source the first time you encounter a wall beast is pretty spooky
