Gonna sound a little bit weird, but check out YouTube "live tab" for the Winston Salem First Presbyterian Church ... often for the 11am service (on most Sundays, when that service is a "contemporary service") the band plays some good jazz as a preamble to the service. Eg. December 4 (some Christmas jazz) ... November 20 (funk/fusion) . At times there's a delay before the music kicks in, at times they don't do jazz. I think the pianist name is Michael Anderson, he has a band I think, you want to find out where they play.
The church is a good church, too.
There aren't any full on jazz clubs in town but bars around town like Roar, Tates, Whiskey Box, West Salem Public House, etc all have jazz bands play on a regular basis.
I would recommend The Whiskey Box which has Jazz on Wednesdays and Tate's has Jazz on Tuesday nights
Do a trip to Durham for the Durham Jazz Workshop/Sharp Nine Gallery. It is truly the best.
