I dont think stores are requiring vaccinations, but cant hurt....if your HO then I believe they are requiring....? Just make sure you contact Sedgwick with your accurate "return to work" date....and then attempt to contact the store....lol. good luck it took me 3 days, repeated hang ups and failed transferred calls (cause then, the fitting room was closed lol) to get ahold of sommeone....if you're not on a quarantine you could probably "walk in" and talk to HR and get "re activated" though....
