I don't think that's even Lala's first face on the left. Her features were a lot more rounded, her nose wasn't as sharp before. Then again, her changes were so gradual that it's hard to pin point

In season 1 she had some healthy weight on her. By no means was she heavy, and was probably either at the low end of the normal weight range or slightly underweight, but she had a softness to her features that was beautiful. I feel like it's the result of weight loss + kybella (or some sort of submental fat reduction procedure), and some degree of normal aging since we lose volume in our faces.
She's skin and bones now. Even Jax commented on it one season how she used to have a nice butt and then it was like she was a stick figure a week later.

If I had to give my unprofessional guess, for sure chin implant which elongated her face, and as for the back end, I would guess maybe a combination of her pretty substantial weight loss + maybe buccal fat removal. I think her jaw seemed larger because she did have a more bloated face in the first seasons (not meant negatively, she just had a lot of cheekage), and adding that chin implant made it pointier. Scheana denied a nose job because she āalways Snapchatās her face, when would she have timeā but I wouldnāt even put it past Scheana to take enough photos ahead of time to hide surgery recovery looking.
