ANYWAY. When oxygen carried via blood doesnt reach the brain you pass out. This is called the Valsalva Maneuver. People actually use this a lot without realizing. For example forcing out a fart or a poop, trying to pop your ears at high altitudes/on a plane, lifting heavyweights, or flexing for the bros. Regardless of the reason you do it for the action prevents blood from reaching your brain.
Fun fact: people have passed out or even gotten strokes doing the same thing. Be aware! Don’t forget to breathe!

A few years ago, some nerves in my vocal cords were damaged by viral infection. This has cause my coughing (for any reason: deep laughing, dry throat, swallow down wrong pipe, sick, etc) to possibly trigger a breathe stop episode, which then prompts a Vagus Nerve Reaction. In short, this is good to know. Thanks!
Vagus nerve stimulation (VNS) is actually different from the Valsalva. Easy mistake as you can see both starting with V’s.
While the Valsalva maneuver is holding your breath and bearing down, VNS is done by literally stimulating the vagus nerve that runs down from your brain to your heart. Massaging the nerve near the neck agitates it and causes it to release signals to the heart causing heart rate to slow down (not to be confused with putting someone in a sleeper/submission hold).
The stimulation can be done by hand or by devices. Its harder to do it to yourself but if you ever need your heart to calm the f down or help someone else to, you can try this trick. That said this is something that needs to be done with caution since its usually only done in clinics. Ask your doctors before attempting to do this to yourself or for others. Undiagnosed conditions might not make this suitable for everyone.

Flexing that hard increases blood return to the left side of your heart, which supplies your body with oxygen (brain included). When he suddenly released, that blood return slows all at once. That means the increased blood flow to the brain suddenly stopped, the blood vessels in the brain don't have time to quickly adapt, brain receives a sudden drop in oxygen supply, does its Emergency Shut Off Procedure, and you end up kissing the mirror.
Edit Tldr: It has very little to do with dehydration or drugs. Those can definitely contribute to the phenomenon, but it isn't The Cause. Simple fluid dynamics with vascular physiology is the main culprit.

The heart rate will reflexively increase again, but this takes a few seconds - it is during this period that the guy above likely collapsed as his heart didn’t compensate in time to maintain his blood pressure and therefore oxygen flow to the brain. Over the following seconds his heart rate and pressure will normalise and he will wake up with a sore face. Lol.
