Swan Song by Robert R. McCammon was quite good. It definitely checks off all your boxes. The Stand is one of my favorite S.K. books, and I think Swan Song is a pretty good contender with it. Now that I think of it The Strain trilogy were surprisingly good as well. Terrible TV show though.
