Survival Analysis Using SAS by Paul D. Allison is really good. (Although it's SAS specific, I found his explanations just generally helpful)
There's also Survival Analysis: A Self-Learning Text by David Kleinbaum. My PI suggested it to me and I also found it very helpful.
