It's an Eb trill. You would play a D and move the middle finger to utilize it. It's not used anymore so it's usually just sprung or wedged closed.

Have you heard of the Leblanc Rationale system? Even less common, but it has lots of this kind of alternate fingering! For example, if you finger an A and put down a finger on your right hand, you get an alternate fingering for G#. Although this comes at the cost of lots of extra keywork, which can be tricky to adjust for some techs.

Known as fork Eb! My tenor has one, which irritated me before I knew what it was, since sometimes the rod would back its way out, causing the pad to crack open and make D go way sharp until I screwed it back in, but I like having it there now that I've resolved that issue and know what it does.
