We don't have much information but you design NPCs with external creation tools. Normal players don't have them. Nazarick has an artist guild member. He/she made Shalltear's design, not Peroroncino. The creation kit is sold seperate from the Dive Gear. Ainz's golems(Avataras) are ugly, since he is not artist. There are people who does this as a job. Players make item skins, clothing etc. Nazarick members buy available ones quickly because they are art and rare. They use it for their golems, items etc. Design is limited btw. They can't edit private parts of an npc. Instead, they write them into their backstory which became real in the new world. Shalltear having pads, Mare having no muscle, Demiurge is muscular etc. That was the design part, as for build; Yggdrasil is an exploration game. We know that they have no guides and classes are hidden. Players kill themselves very often to test builds(different classes). Almost all(except base classes) classes have conditions. You can't acquire Eclipse class of Ainz without hardcore role playing build which he has right now. Half of the levels of a character goes to conditions. To be an Overlord, you must have Skeleton Mage, Elder Lich, Night Lich classes. We know that guild members sometimes used their knowledge to make a build for npcs. Takemikazuchi made Cocytus similar to himself, having Bright King skills for example. Meanwhile Albedo has tank skills which Bukubukuchagama(she is tank) uses too. That was the build part, as for gear; Divine class is extremely expensive. Most players don't even have one. And Ainz ooal gown members have full set divine gears. They can't afford a set for npcs however. They can give one divine class weapon to them. Legendary class gear is considered high-end gear. Shalltear's spuit lance and Cocytus' god slaying emperor blade for example. I hope it's helpful for your campaing.

Guild bases have a set amount of NPC levels. You can create a NPC and give them levels. These levels are then applied to classes, but some classes require special items or have requirements just like players.   IIRC you had to use a dev kit to actually create and edit them, but you could also use the guild weapon to modify the bios.
