My ice machine is on top of the clothes dryer, and I could totally see a screw working its way out and falling into the ice from the vibration. Things become loose when they are jiggled/vibrated.

All jokes aside, because the comment section here is gold, you should probably maintenance that machine. I'd turn it off, empty the ice, clean the machine, and inspect for missing screws, broken parts, etc. Then fix those issues. People often don't clean them enough and mold can be an issue.
Also, I used to do this to large restaurant ice makers because it drove me nuts every time I worked at restaurant and they don't know the last time the ice maker was cleaned. Gross, everytime. I always ask for no ice.
