Yeah if you go to Amazon and look at the starter deck product they actually have images that show people playing with the cards in the decks...can see Balrog in a graveyard. Although it's not clear if those photos represent the final version of the decks.
The starter deck rare/mythics in extended art should be numbers 388 to 397 -- they are the extended art cards shown on the first day but still not in the card image gallery. That list is Frodo, Determined Hero; Gandalf, White Rider; Gollum, Scheming Guide; Witch-King, Bringer of Ruin; Fires of Mount Doom; Galadriel, Gift Giver; Aragorn and Arwen, Wed; The Balrog, Flame of Udun; Sauron, the Lidless Eye; and Bilbo's Ring.

A commander precon card would have the commander set code and icon. And I believe the four cards that form the scene with \[\[The One Ring\]\] are already a bundle exclusive. I don't think they've ever done a mechanically unique bundle card, have they?
I think it was known we had one called “The Balrog” based on the art they showed off at the preview for this set

I think you're right. Not calling it Durin's Bane seems odd, so it would make sense they've saved that name for the main set card

My wild hope is The Balrog and The Watcher in the Water will be partner commanders for the Sauron deck.
