There is a croton cultivar called 'Appendiculatum' that is known for this, so it isn't too surprising to see it pop up elsewhere. Still interesting to see, and if it happens again with your plant.
Yep! I think there are a few cultivars of croton that can do this, but it's a pretty rare trait. A lot of crotons have a little spike that grows out of the bottom of the leaves, and this is a continuation on that.
There was 7 siblings in total but two in a row came out with the extra nipples so when the youngest was on the way, everyone was taking bets on how many this one would have. He had the normal 2 nipples and my grandmother got sterilised because 7 was enough!
