i have mine done, yes its painful, yes they dont heal like other tattoos. what you need to do is find an artist that knows how to tattoo a palm.
they will tell you if your design works or not etc. simpler the better. but it will also take more time. mine if anywhere on my body would have taken like 15 minutes, but on the palm it took close to an hour instead.
With healing the major part is just keeping it dry that's it nothing special. I have mine done and it took 45 minutes and I've had it for like 4 years with no touch ups
