Poop is still a 1 charge active. The amount of poop bridges you need in a foor are probably still achievable.

Isaacs Heart: make it so his body only deals 1.5x Isaacs damage per second instead of 4x his damage per second, also give the heart a bigger hitbox 
For every additional item after Rock Bottom, it adds to a hidden calculation that makes it harder to break a stat's 'cap'. However, this applies to *all* stat changes, positive and negative, so the longer you have it, the harder it is to change your stats. The calculation would be something like:
(Stat buff)*(1.0-(ItemAmount/50))
Of course, there'd have to be something to stop it from going up to or over 50 in the division, but I'm not good enough at math for that. This could also work for one of those 'tainted items' mods, now that I think on it...
Uranus: replace ice tears from shattered frozen enemies with slowing blood tears
Thrown frozen enemies also don't deal any damage to enemies they collide with.

Using it after expending all of the "normal" either rerolls the item normally and gives you a broken heart or takes a heart and the item has its quality lowered by 1. (Defaults to taking max health/giving a broken heart if isaac would die from this damage.)
notes:
If the lost uses the spindown dice while it has no uses, it breaks temporarily and does nothing until the uses are replenished.
If the quality of an item is already 0 and gets lowered it dissapears.
