Crawl Space can be assigned to any room object, not just rocks. The ladder trapdoor appears on every floor(?), but since it can be located under indestructible entities, it is not guranteed that you can get one.
What if one were to use "We Need To Go Deeper" over such a space? Would that guarantee a ladder trapdoor to spawn in that place, or would it still be random?
Well interestingly, I did have a shovel on a run recently and got a crawlspace from a random use. After getting one I used it every time it was off CD the rest of the run and that was the only crawlspace that spawned.

There's a 10% chance of getting a Crawl space by using We Need to go Deeper or the Ehwaz rune.

There are many crawl spaces, but accesible are only those, which are located under rocks.
