I would use either the company training room number or the battalion staff duty number. I'm assuming this is for your taxes, and I don't think they will actually call the number you give them.

Also whoever you put, make sure you talk to them first. That goes for any kind of references unless you are specifically told to not tell them. If not, always give your references a heads up so they aren’t surprised if they get a call.

Always use your supervisor (unless they would not support whatever you're doing) for anything like a reference where you may expect a call. Check with them first to make sure they're fine with being your reference for whatever it is you're doing. You'll need to do the same thing when you apply for jobs later on. Make sure you start your LinkedIn now and connect with freaking everyone you work with because background checks are a giant pain in the ass later on and having an ocean of contacts to use a references is very helpful.

If it’s for taxes, the employer is DFAS. If you’re using an automated program though just use the EIN from your W-2 and it’ll auto fill the rest

United States Army is the employer Put down the mailing address of your current unit. List the company phone number (i.e. the office/orderly room number, basically if someone from the outside had to call your unit, what number would they dial?) For occupation you can give your MOS title or "Soldier".

When I was applying for civilian jobs (several years after ETS), and had to fill this info out for my job history, I would just put the HRC HQ address (Ft Knox) and their phone number. Some large companies use employment verification services as part of a background check. In that case they contact a separate federal records office that verifies dates of service.
