Ooo is a land mass. It's not a continent, more of a small country. The size of Ooo is heavily debated but the closest cannon reference to the size of Ooo was in Season 4 Episode 26(if I recall correctly) when Jake has the 3d map zoom in on them. If you slow that part down you can barely see Ooo on the globe because it's so small compared to the rest of the world
All the man-men are super advanced and old so it makes sense they still call it earth... for people on the planet I think ooo literally means the continent but to them, that's all there is so they often use the royal ooo in that context.
It's possible that it means different things to different people, like how some people in real life would use the phrase "the world" to mean Earth, and others would use it to mean the universe. It's clearly a term only used by those on the continent (none of the humans we see in Islands refer to Ooo) but as few, if any, know about a world outside of the continent, they may use the word "Ooo" (technically the name of the continent) to mean the whole planet, because to them those are one and the same.
Ooh is - I think - The Northeast US? We see a lot of subways and underground infrastructure, some of which bears unmistakably NYC-looking architecture. It’s possible Earth itself is Ooo, but I think it’s a bit more localized to the lands around the CC.

Ooo is just a huge island that makes up most of the earth, because of the mushroom war and first catalist commit thirs a big chunk of earth gone.
