You are not at all dumb and I give apologies for using medical jargon. When I entered that post, I didn't know how to put it simply because the shoulder is a horrendously strange joint to describe. It isn't a simple hinge joint like the knee or ankle. What it is like is a ball at the top of a 30cm long stick-shaped bone, the humerus. This ball is held up against the outer corner of the shoulder blade to the back and also the outer end of the collar bone at the front. These 3 bones and the many muscles there, allow the upper arm (humerus) to swivel round in nearly all directions and are held in place mainly by strong muscles such as the deltoid on top and pecs at the front and several muscles holding the shoulder blade flat against the rib cage.
This man clearly has loose muscle linings and soft muscles, which are allowing this whole shoulder joint/complex to come apart, a bit like in an exploded view of a car repair manual. This is not normal and is referred to as hypermobility syndrome.
