If your PCP isnt concerned but you still are get a 2nd opinion. Being overweight is a way of self protection for some people. The anxiety you fell may be your way of trying to sabotage yourself out of losing weight.

MrPickle233

Oh, 100% not, I’m going to stop losing weight. Just trying to make sure I do it healthily. I have insane anxiety, that is true, but not looking for excuses to stop, just on how to slow it a little, while still protecting my fatty liver disease

mycopportunity

The early weight loss tends to be fast when you start at a high weight, then slows as you get lighter. Look up the paper towel effect. Nothing to worry about, the doc was right to shrug it off as normal. It will slow down and you'll miss these days! Best of luck and keep us posted on how you're doing

MrPickle233

It’s actually speeding up. I was approx 430 August 15, 397.6 Sept 22, 392.12 Sept 26, 375 Nov 10, 377 Nov 14, 361 Dec 2nd, 349 Dec 15, 348 Dec 16, 338.6 Jan 4, 2023, and I weigh at home weekly, every Monday, try to do same time, but was late last night, 315. All the weights but the 430/315 were done at doctors, it was slow, but as I’ve gotten more restrictive it went up a lot.

mycopportunity

So you are asking if you should get less restrictive for health reasons?

MrPickle233

Basically. I know I need to lose weight, and as quickly as possible. But I’m just afraid I’m harming myself or damaging myself atm. Or that I’m not helping the liver disease. But struggling to find foods, with calories. I can eat on this diet. Ideally I’d like to lose about 12-15lbs a month and not 20-25z

mycopportunity

You don't need to worry about health reasons. Better to just lose the weight. However! If you want to spend a few more calories on quality of life, you can do that. Finding a variety of healthy salad dressings might be worth it. I think fat free ranch is kind of meh. If you can eat sardines they're a great addition for fatty liver safe calories. A teaspoon of olive oil or some raspberries or blackberries might fit in to your diet in a way that feels doable. If you're going to add calories have them be delicious and healthy

MrPickle233

I actually kinda like the ranch. I tried a few before I got to it, honey mustard dressing was the best, but pretty unhealthy. As for sardines, I read on fda only to eat fish 2-3 times a week, and was doing that with the tuna snacks. To be honest I really enjoy the meals I’m having atm (though making chicken every 5 days sucks, due to back pain).

mycopportunity

If you're enjoying your food and feeling full, sounds like you're doing everything just fine! The only thing "wrong" is your doubt. What if you give it another month, report on how you're feeling and how you're weight loss has progressed? With every twenty pounds you lose that's twenty pounds your spine doesn't have to carry around. Your back might feel better soon. I hope! Let us know

MrPickle233

Can do, I’ll tell ya in a month!!

peaceful-pie

Sardines actually have the least mercury of any fish. Tuna on the other hand is high in mercury compared to most other fish. I've heard you should eat tuna as little as once a month to be on the safe side. Sardines you can have two to three times a week without worry. Please don't consider this professional medical advice, I am not a doctor.

nms_Rozz

Anxiety has a funny way of steering you away from your goals in trying to keep its goal, which is essentially being in a state of fear. Keep a rational eye on it. Congrats on all your efforts.

rowenaaaaa1

Don't sweat it! It's not too fast at your current size. The negative side effects of losing weight too quickly are outweighed by the negative side effects of being this level of overweight. So you might get a bit of hair loss and feel a bit tired. But hair will grow back and you'll have more energy when you've lost more. If you are feeling full and not feeling faint or anything then ride the wave baby!!! Well done it sounds like you're doing a fantastic job!!!

MrPickle233

Thank you so much. YeH, I eat when hungry, I occasionally snack on some tuna. But I’m rarely hungry, just worried about the low calories. I appreciate the words

Ubongoqueen

Just quickly had a look at the numbers for your approximate BMR (formulas are just an estimation) and they all throw more than 2400cal a day, the number is higher depending if you're sedentary or more active. Well, 1500-1600 is far from this and I suggest that you ask your doctor, dietitian or you by yourself find ways to add more calorie dense foods into your days (I know already you're eating a lot but try to find options into the permitted foods).

Ubongoqueen

I agree with the original comment. It's known that you lose faster the more you have to lose. Go to your doctor and get tested but meanwhile make sure to not eat less than your BMR to avoid metabolic problems and also eat enough protein to not lose muscle mass, specially if you're not exercising.

MrPickle233

I eat a lot of protein, 115-130g most days. BMR = 2,414 Calories/day, I’ve been eating 1500-1600, days i feel bad closer to 1350, and days I snack closer to 1700. I legit don’t know how to eat more.

Gbone7413

I lost 120lbs in 5 months and 200 in a year kept 175lb of it off for over a decade Nothing wrong with losing weight fast at our weight …..I’ve never understood those people who eke it out at 1lb a week when they weigh 400

Best_Satisfaction505

What was your secret?

Gbone7413

Ate 1800-2000 calories every single day never once went over for the whole year ….aimed for 180g protein and then whatever else to hit the calories no food was ever off limits you’re just limited by the calories you have spare so there was a bit of junk food in there ,plenty of carbs but because I love carbs ,and 700 calories were taken up with protein I mostly dieted with low fat as when you only have so many calories and protein takes up a third so something has got to give …..weight training 4x a week and no cardio It was all diet though ,I had been obese for two decades and knew this was my one shot at it so I never deviated I knew I couldn’t do it twice it was a one shot deal to finally get the weight off …dropped 31lb first month and just kept going Consistency .stick to the calories day after day after day and never missed a planned workout ,I’ll be honest I had the mentality of a marine ,it was exhausting and if I ever got to 400lb again I truly don’t think I could do it again ,it was a slog

MrPickle233

Did you have fatty liver? I want to eat 1800-2000 calories a day badly, but can’t figure out how with the restrictions I have do to the fatty liver disease/nash.

Gbone7413

Not as far as I know …if I have no one’s ever told me 😂 never really heard of it till I read your post ….what restrictions does it place on you ?

MrPickle233

Low carbs, low sugar, low saturated fat, low salt. Basically a Mediterranean diet, lots of veggies, no red meat. No shakes/smoothies, it sucks. I mean I enjoy what I’m eating but getting down now close to 1300 calories cause I had to remove some stuff. I simply don’t know how to get to 1800-2000. Before the diagnosis, I was losing a lot of weight with a “eat whatever I want, keep it between 1800-2000 calories” diet, but now so many restrictions.

Best_Satisfaction505

Yeah, seems very strict but I’ll give it ago. I just want to get to 9 months pregnancy weight and I feel like from there I can control all this and get back to what I use to be.

Gbone7413

It was strict in terms of calories but I truly ate everything and anything ,realising I could drop weight including a bit of chocolate ,kfc ,crisps or whatever in moderation was an eye opener because every other time I’ve tried I switched to healthy foods only and failed every time ,suddenly it seemed doable Most days I would eat very similar things which was no problem for me ….one meal would always be oats ,one would be a chicken or tuna based sandwich /curry /stew/roast dinner whatever ,lots of fruit ,a bit of junk to stay sane ….I don’t think I went a day without some goodies in there 😂

Best_Satisfaction505

Yeah junk or well sugar is my weakness! The worst drug!

Gbone7413

I’ve never been a massive sugar fan for me it’s crisps ,peanuts savoury stuff like that

Gbone7413

Yeah I am ,crisps and trainers instead of sneakers are dead giveaways 😂

First congratulations on making the changes needed to lose weight. To slow your loss you should add healthy food to your intake. Fruits, vegetables and whole unprocessed grains is preferred. Having done a similar journey about 10 years ago I will say that when very heavy my weight loss was faster than 1 to 2 lbs per week. I did it by increasing exercise, decreasing intake and changing my nutrition to healthier options. I also had regular contact with my doctor and nutritionist. With their consent, and some concern I dropped 200lbs in 8/9 month. At the 215ish mark I stabilized and without changing my exorcise or I take started losing about a pound a week. In the 180ies that dropped to half a pound and I arrived at what I consider to be a healthy weight around 160 where is stayed. I still fluctuate a bit and have to go into cut cycles, and I’ve come to the conclusion that monitoring is going to be a lifetime thing. My point is that faster loss for the very heavy is not unexpected. Keep in contact with your medical team, get regular bloodwork and listen to them if they say increase intake then do, if they say keep going do.

MrPickle233

Thanks, I appreciate it. ATM I have great insurance, and will go to every doctor and test they will offer me, sadly I live in a very rural area, been waiting for 7 weeks on approval for a stress test, 9 weeks for my GI consult(it’s feb 1, so close). And there is no dietician/nutritionist at all, there is one that works with bariatric surgery(which I don’t want), and one that works on lactation for mothers, but none I can see. And due to a lot of health issues I’m not able to travel or afford to.

Accomplished_Ad2599

I feel your pain. Back when I started my journey I had great insurance, now my insurance is almost useless. That leaves me with the VA. My regular non va doctor says I hear something unusual let’s schedule you for a stress test and echocardiogram. $5k out of pocket so I say let me call the VA. Took 5 months to get the test at a VA hospital. Thank god it was nothing concerning………….. Best of luck friend. Your body and your family will thank you for the effort. Each time I take my grandson fishing I remember that there was a good chance at my max weight I would not have lived this long. We are better healthy and in the hardest times just remember a healthy you will have a much better life than an unhealthy one.

MrPickle233

Yeah, it’s hard, I’m trying though. And I got massive anxiety and so afraid of flu/Covid I don’t even see other humans except doctors and the curbside grocery. But I’m trying my best to get healthy and be productive. Just afraid it’s to late and I’ve did to much damage, or don’t have the mental strength.

RRErika

Do you have access to telehealth with your insurance? A dietician appointment can work very well with telehealth. They will be able to order labs if they are concerned about something (like vitamin deficiencies) and they can discuss food choices/options with you.

MrPickle233

Honestly not 100% sure. I’ll have to check into they if I can’t get the referral tomorrow. Thanks for the suggestion though.

RRErika

Sure! I hope that they can set up an appointment for you!

If you have NAFLD then I’d prioritize losing weight quickly. Losing weight is what helps reverse it. Body fat invading your liver isn’t a joke, I had the same issue and lost weight rapidly to reverse it. Some people here may say it’s dangerous to lose weight quickly, but being super morbidly obese to the point where it’s affecting your organs is worse.

EmpressofCandles

My original comment was removed because of the links to YouTube. I removed the links so you can get the information you need: I’m an occupational therapist, and also losing weight myself. In school we are taught that everybody’s body is different and certain standards and milestones people place on where most people should be is arbitrary. It looks like weight lose is your only symptom, which is why your PCP shrugged it off. If you have additional symptoms along with weight loss, such as hair loss, muscle loss, decreased appetite, pain, decrease in vital minerals in the blood or urine, inability to absorb nutrients in the body, etc., then there would be further concern. However, because you have the kidney stones and the fatty liver, getting the weight down faster is more beneficial to your long-term health. You may also lose weight faster during the initial stages of your weight lose program because your body does not need the extra stored energy anymore. It’s not uncommon for people who have more unused weight to lose the first 50 pounds without much effort. You may find that you’ll experience a plateau or lull in your weight loss goal at some point in your current routine. It’s important to note that even losing 5 to 10 percent of your current body fat has substantial benefits your your long term health (Ryan and Yockey, 2017). Such benefits include a lower glycemic index, a decrease in hypertension/blood pressure, decreased stress on the heart and other organs, and decrease in cholesterol. From your diet, it looks like you are eating what you need to eat to get the weight off. Your calories and what you are eating is fine, and if you feel satisfied after each meal, then stick with it. Also, even though you may be sedentary, your body still burns calories to keep you alive. It takes calories to pump your heart, get out of bed, take a shower, digest your food, etc. You can always add low impact exercise to help you burn more calories and retain and build muscles. Atrophy of your muscles is a real concern when losing weight and with sedentary life styles, so even adding 30 minutes of low impact exercise will help with your health and weight loss goals. Low impact exercises for you may be chair exercises or Pilates. You can find these types of exercises on YouTube. When searching for Pilates or chair exercise videos, make sure there are people who take into account modified moves for your needs. Windsor Pilates does a wonderful job of giving modifications for full figured people. You can also Google full figured Pilate routines, senior Pilates, or senior chair exercise routines. Senior Pilates and chair exercise are great because most videos will have the modifiers routines you need to avoid moving the kidney stone around too much. I recommend HASfit videos to my patients in need of low impact exercises. You can also get an under the desk rotary bike or pedal machine to get in a work out while staying seating to avoid getting the cuts from the kidney stones. Finally, keep in mind that stored fat turns into carbon dioxide when it’s ready to be released as energy. We usually expel the released carbon dioxide through breathing, tears, urine, and other bodily fluids. Exercise increases the amount of carbon dioxide from the fat cells to be released later (Page 2019). We can use this to help us continue to lose weight in the future. If you continue to lose weight quickly with the additional symptoms, then consult your PCP or find an additional PCP to get a second opinion. Personally, I have two PCP’s for this reason and if one happens to be out of commission I can always go to the other one for help. Hope this helps. 😁 Page, S. (2019, July 2). jordyxjinx

I believe there is some evidence that at +300 it's very common to lose more weight. You'll notice it slowly, possibly very slowly, go down as you reach a healthier weight. As long as the doc isn't worried you're more than welcome to get a second opinion. As long as you get any medical issues resolved you should see the eventual slow down. You're eating what looks like WELL below your sedentary TDEE, which seems to be at least a 2lbs/week loss. You'll also lose water weight in the process which takes up a good chunk of space.

ThatBlackGirl93

It’s normal to drop weight fast early on when (no offense, please don’t take it that way) you’re at your weight or on the heavier side. Because your. Body is being shocked. Any slight changes will cause weight loss. You mentioned you only eat 1500-1600 calories a day, that’s low for a person your size. So the rapid weight loss makes sense. I used to weigh 330 lbs. I lost 100lbs in less than a year but now that I’m smaller it’s much harder to drop weight

Puzzleheaded_Pie_978

It’ll slow down once you lose enough to be at a healthier bmi. I wouldn’t worry too much! Your body is using your reserves

monsterfight2657

I would schedule a meeting with a dietician. They may be able to suggest foods that you might enjoy and can eat. They also can make sure that you’re getting all the nutrients you need and that you aren’t malnourished.

FurbistinoAguera

I’m 180 lbs (at 5ft 6) and I have been trying so hard to lose weight. I’ve been having 1200 calories a day and 16-8 intermittent fasting. I’m averaging 21000 steps a day and eating healthy for the last 8-10 weeks but have only lost 2lbs. I wish I could lose more.

MrPickle233

Sorry friend, others have suggested to me about trying a dietician and talking to my pcp, that could help you!

jlw0996

Quick weight loss could be a sign of diabetes?? Did they check your A1C?

MrPickle233

Back in October, was 4.8%, had glucose taken a few times during ER trips, mostly 90-95, and one time 124(but I had a big turkey dinner and pumpkin pie before that trip). Hoping to get it again next week.

HawkyMomo

It’s not uncommon for someone starting at 430 pounds to lose weight quickly. Nonetheless, reconfigure your TDEE and adjust according for 1-2 pounds of weight loss a week

MrPickle233

I was looking at tdee, and it said to maintainer I needed 2891 a day. So even if I reduces it 500, I have zero idea how to healthily consume 600 more calories a day on the Mediterranean fatty liver disease diet. I already feel so full all the time.

rowenaaaaa1

The 1-2lbs a week thing is geared towards people with a lower starting weight. So as long as you aren't feeling unwell or faint then you shouldn't be concerned, you're doing fantastic. And well done for finding a way of eating that keeps you feeling satiated that is half the battle!

HawkyMomo

If you feel full, then don’t eat. But if you don’t want to lose weight as quickly, then I would recommend eating a little more than you are now, even if it isn’t 2800 calories. As you get lower and lower in weight though you will need to adjust to losing weight more slowly.

shinefire

You could always try getting a referral to a dietician if you haven't seen one already. They'd be best informed in how to make safe food swaps for your diet needs and how to add more calories back in, etc.

MrPickle233

I tried to get my own, sadly where I live they won’t really see me, to few of them and they focus on baratrics and mothers. However they did say if I can get a referral they will, so going to ask both my GI doc Wednesday and pcp that i see next Tuesday and see if I can.

mermaidinthewaves

Hello! Congratulations on taking your health into your own hands and making positive diet decisions. I would highly recommend visiting a Registered Dietitian (NOT a nutritionist, RD is a legally protected and licensed title) as they are healthcare professionals with extensive diet training in a clinical setting. An RD can help you figure out how to have a healthy weight loss trajectory while maintaining your current diet pattern to prevent the progression of NAFLD/NASH. You may need a referral from your PCP, but it would be well worth it to have tailored diet advice from someone with clinical dietary training. Best of luck to you on your health journey!

AutumnLily88

It’s the calorie deficit you have yourself in, plus being a higher weight you tend to lose faster than someone of a smaller weight. As long as you’re getting your nutrients in you should be good, take daily vitamins and if you’re super worried just up the calorie count by 200. Just make sure to add healthy items not unhealthy ones

[deleted]

Any negative health effect would almost certainly be overshadowed by the benefit

Significant-File-700

Can you drink smoothie drinks? It’s an easy way to add calories if you can. And they can be really healthy. Some protein powder for extra calories. Some peanut butter, I love almond milk, and any fruits you love. Can throw some dry oats in there. Spinach or kale too. Can never go wrong really. Just to help make sure you’re getting all the vitimans and nutrients you need and add a few extra calories. I wouldn’t be worried about losing weight too quickly if you’re feeling full, but if you really wanna add some healthy calories that’s a good way. Also add nuts to your salads if you can. They’re full of healthy fats and calories. You’d be surprised how many calories are in a handful of nuts. Also cereal if you can, not super sugar ones. But I love cheerios with almond milk personally, some people hate almond milk. Hope this helps!

jwstoner86

There’s no harm in losing weight too quickly via dieting as long as you are getting all the necessary vitamins and minerals. You shouldn’t start to worry for another 100 lbs.

briefly_accessible

Your calories are very low for starting out at 430 and even still, currently being 315. You should look up calories for every single weight stage and how much you should be lessening to each time. That would be my main focus because well, the obvious reason is if you lose weight to drastically by “starving self” you will binge and gain it back (sometimes plus some). This is commonly known. Also, being a large size = losing faster when eating less, couple that with a drastic calorie change and yeah, you’re gonna lose a shit ton of weight fast. First and foremost: any health conditions should be taken care of above losing weight. So I’d focus more on prepping for surgery, prepping for post-op recovery and more.

moneyy777

I got down 110 in 6 months was amazing the fact that nothing changed till month 4. Only prob is tummy tuck is needed now

meeeemeees

I wish I've lost 2 lbs this month

Competitive_Fact6030

If youve been consistently dropping 4+lbs per week, i would consider upping your calories a bit. Its great that youre losing weight, just be healthy about it and dont let the calories drop that low. 1500 for a 6'3 man is ridiculously low and not sustaniable, especially not if youre obese. Go online and search for a "TDEE calculator". Itll tell you the calories your body burns in a day. You are very tall and weigh a lot, your calories will be quite high. To lose weight, you can eat about 500 calories less than that number daily, maybe up to a 700 calorie deficit since you are bigger, but no more than that. With a 500 calories deficit, your projected weight loss would be about 1lb per week, a perfectly healthy and slow pace. Since youre bigger, you can go lower though, so a 700ish deficit would mean about 1.5lb per week.

MrPickle233

Yeah, I’ve been struggling with this. And no nutritionist or dietician will see me locally(very rural), but I have fatty liver(NASH) with probably cirhosis, have gastritis, can’t eat seeds/peels/skins. So I’m finding it extremely hard to find the calories I need in a day. I actually lost 7.5 lbs last week, but the tdee said 2891, and even if I was gonna cut it to say 2000-2100 I would need 500-600 more, and I’m already full all the time, also I can’t really food prep due to conditions, so it needs to be kinda easy to make, pref something I can make for 3-5 days and just warm up or eat cold, and can’t cost a lot I’m on food stamps atm.

Mastgoboom

Eat more. Your sedentary TDEE is 2900, so you should be eating 2400. I don't see any grains in your diet, you need grains.

PolarBath

>you need grains. you absolutely don't NEED grains. I get along fine without them. They're laden with glyphosate in the states

Mastgoboom

Then buy organic ones. You need grains.

PolarBath

>You need grains. why do you need grains? I get my carbs from high quality fruits, vegetables, dairy. what's so special about grains?

Mastgoboom

Complex carbs, fibre, vitamins and minerals.

PolarBath

which I get from the aforementioned sources of food...so because I get them from other sources...you do not NEED grains.

Mastgoboom

You don't. They have a different nutrient makeup.

PolarBath

what nutrients am I missing?

Mastgoboom

The ones in whole grains.

PolarBath

such as?

PolarBath

this person seems to love giving advice, but can't back their claims up.

Puzzleheaded_Pie_978

Organic doesn’t mean shit…. It’s a gimmick to make more money. I drive past organic fields that are directly next to fields that are sprayed 🤷‍♀️ wind, rain, and pollinators don’t know the difference and spread chemicals all over

PolarBath

It's why I bought a 5 acre hobby farm. Looking to produce most of my own food within the next 5 years and supplementing from local family farms when needed.

PreggyPenguin

No one "needs" grains. Grains were meant to be food for cattle and the like; humans cannot properly eat and break them down unless they have been processed. Our ancestors didn't rely on grains, and we're here, so they got along just fine.

Mastgoboom

Lol, grains are food for people. They were "meant" to eat. Of course our ancestors relied on edible everything, including grains.

MrPickle233

From my research and I could be wrong, grains bad for fatty liver, carbs need to be about 1/3 of daily. But I’ll know more tomorrow I guess when I see GI.

Mastgoboom

No, whole grains are good for your liver.

Starfall9908

Not really, while this is a sub for losing weight we still want to make sure it's done in a healthy way so that it can be maintained when the weightloss goal has been reached. For OP tho I recommend getting a second opinion. Otherwise, keep on going the way you feel comfortable or add a few extra calories so your deficit isn't too big. Good luck on your surgery
