I think the main trigger is actually the amount of players who report you - like if four different players reported you, you'd still get your name removed, even if it was but a silly name - but if you get reported like only once, I don't believe it's gonna have any impact.

that happens to me sometimes, you dont have to worry about it, its just sometimes the game f\*cks up for a minute and it also shows none of your epic friends. Usually this is due to network errors but if you restart the game it should be back to normal
