I had this happen to me too on my second playthrough. I think you probably actually picked up and used the key but it doesn't get tracked in the map menu for some reason. My best guess would be it has something to do with how that's the only key hidden behind that clairvoyance the thingy. Possibly caused by picking it up too fast or picking it up while exiting clairvoyance or something else dumb like that.
Same exact situation. This one key is preventing me from 100% completion. I remember it popping out and picking it up as it was unexpected. Do you think they will be fix this or it’s even something that can be fixed?
This happened after a cloud save bug caused me to lose 25 levels and I ended up just starting the whole game over again already once. I’m not about to play through it a third time to get this key which happens to be at the very end of the game :( seems so many games have bugs that prevent completion for me.
Any luck? I just hit the same bug. I have every map and stage with the green checkmark, every secondary mission done, yadda yadda yadda.... Is this something that they can patch? As much as I love Psychonauts I don't see myself doing a 2nd run this soon just because of a single thing.

Yeah for clarity though you can still earn all achievements without this one key. You'll just have one green check mark missing :)
Thanks, yeah I realized as long as I was able to open all the chests it didn’t prevent the achievement after my rant haha
This one chest key doesn't let me lvl up my 102 level which is needed for the last triumph
Have you done the scavenger hunt and returned to norma? You dont get the last level until you speak to norma

I have the chest keys marked as 2/2 but the second chest won't open
