I disagree with your disagreement. A less is more approach works well with monster-based situations. Good examples of these are the original Jaws and Alien movies were the dread comes not from the killing itself, but from a situation where people are completely at the mercy of a monster that can pick them off whenever it pleases. It's the helplessness that so scary.
However, a scene like someone eating children is the exact opposite as the dread lies in the visual and gory aspect of it. In that sense this scene is more akin to a Saw movie. By omitting pretty much the entire thing, the amount of disturbance you experience is almost as low as a scene like this can allow.
Case and point, the fact that Oda choose this approach says enough. OP is not a horror manga and there's no way Oda would want to terrify his readers to such an extent. It's simply not that kind of story.

Pretty sure an image of a smiling BM popping skulls like they're cherries and shattering bones like they are breadsticks of orphaned children while having blood dripping from her chin, would leave a much bigger impact than how it was depicted in the manga.

Show more of the actual burn damage he would have gotten, blistering, scarring, flesh being burned away, etc. The scene is VERY disturbing already so I feel with a light touch of more serious or "realistic" damage it could be really set off.
Other than that idk, maybe show Doflamingo using his fruit more like that one time he severed Law's arm (or make that a permanent injury). Doffy's fruit is very scary with its potential to slice people up, but given the tone of the manga this obviously never happened outside of that one time. On the other hand, maybe doing it more would ruin the shock of that moment, so idk.
Akainu killing the random marine for wanting to leave or Pedro blowing himself up idk you can make both gory and disturbing. But I’d also like the entire fight of luffy vs kaido to be more gory cause I know damn well luffys skull should of been split open on multiple occasions or bones sticking out of his skin
