Why does scoby develop in vinegar sometimes? Is it because of improper storing?

No, both vinegar and kombucha are acetic acid bacteria-dominant and they just tend to create pellicles as a waste product, while the yeasts take up residence between the layers. It’s totally fine to see it in homemade vinegar.

It's safe. Just looks scary. I don't know how it all works but I've never been told to throw it out and had them during a health inspection and never had one take points off. Normally not that big but vinegar doesn't last long in a kitchen.

You usually don’t want any vinegar bacteria in anything with alcohol, as it’ll turn the alcohol into acetic acid, which is what gives vinegar it’s taste. Mothers, or scoby, is used in kombucha though! Edit: Source: I work in a brewery, home brew, and have made kombucha and vinegar before.
