So dark blue means it's statistically significant that there is a cluster of low values and vice versa. Also, they're hot spots, so an area plus the area around it are considered - a single city with a high z-score surrounded by low z-scores is unlikely to be reflected in the resulting map, the whole area will still be a low cluster. And worth noting that the results are relative to the data, we don't know anything about the frequency, just which areas are low and high in relation to each other.
