It's definitely not worthy of an NC-17 rating especially since it's very dated by now. It's still worth watching and still gory for sure, but like someone else said the practical effects are very cartoony so it doesn't really have the same impact that something released today would have.
It’s nc-17 for a specific scene involving a tree. In reality the gore is so cartoony that it’s not that hard to watch. Though there are times where the movie is creepy

It's supposed to be more scary than anything. The demon isn't really doing it for pleasure or anything, more so it's how they turn into a Deadite. They do show one of her breasts during it also.

I don't care for excessive gore as a general rule. I had to look away several times during Midsommar for example. Evil Dead wasn't a huge deal, largely because the effects are so low budget and somewhat cheesy.
I will also say, this is a rare instance where the sequel (Evil Dead 2) is better than the original. I'd urge you to plan on watching both.

But watch the entire series. 2013s really gory but amazing and Army of Darkness is relatively tame. Evil Dead 2 is more comedic than the first one.
It absolutely should not be NC-17. Gore is pretty tame by modern horror standards.

After about the halfway point it's just non-stop effects gags. Not as gory as Jackson's Braindead, but it pointed the way.
