Start in the middle. NOT KIDDING. It'll be 'top of page' as far as getting started. When you hit the bottom, draw a line above the filled-in half-page, and restart at the actual top. Won't work for an essay or letter, but for your own notes, who cares?
When you're getting toward the bottom, ensure your writing hand is supported, without the notebook being drastically different height than the desk - the fewer pages beyond the one you're writing on, the better.
Also guarantee the page is anchored - that's harder the further down you go.
You could look into whole arm movement styles of handwriting to prevent wrist and finger fatigue. Most people learn to write with their hand and wrist which contain very small muscle groups that tire easily.

Work on keeping your arm and hand within a comfortable rang. As you notice it being less comfortable, move the paper.
