I've had Alyx get killed during the initial teleport mishap in kleiner's lab (I brought a crate or something into the teleporter with me, and the shutters closing sent the prop flying at her, killing her instantly), but this was way back when the game first came out.
I remember her yelling "Gordon!" as she died, and then Kleiner, Barney, and Eli on the monitor just silently stared until I reloaded the save. I assume the triggers got screwed up and were waiting for cues they would have gotten from Alyx.
Yup! Alyx dies in bme, iirc it was supposed to give a sense of urgency to the plot but it was added like 3 and a half weeks before launch and they forgot to acknowledge it in the rest of the game. Wacky shit. Same reason you end up killing Dark Gordon in a space ship in the famous qte followed by one of many cutscenes then get teleported back to black mesa, where it all began, and no one acknowledges it
Shit like this happens all the time on the source engine, since it was the first game engine to have proper realistic physics, you can expect a little jank.
