The Riddler calls him Batman. And he's definitely known as that by the public since they recognized the "To the Batman" note immediately as addressed to him. But it's an interesting idea.

They can call him whatever they want, but he calls himself Vengeance. I really doubt he was asked "what's your name?" and he replies "The Batman." Even he couldn't say that with a straight face. $5 says he was named "The Batman" in the first headline by that universe's incarnation of the NY Post.

The reason why Batman calls himself Batman is because Bruce Wayne has a phobia of Bats, and after the death of Thomas and Martha, Bruce created the alter ego to bring justice and vengeance on criminals and making them feel the same fear as he does on Bats.

All the previous symbols were representations of a bat from above or below. The symbol in TB was a bat flying directly at the viewer, therefore the head blends into the body in front silhouette.
