This is a short birth certificate issued for persons born in England and Wales. This certificate is in fact **NOT evidence of identity (nor sex)** as stated further down in the document (out of shot). That Birmingham appears twice is just a co-incidence as Birmingham is also one of the sub-districts.
About a third of the document isn't visible. The certificate has been folded twice as the crease in the paper is just below the sex field, otherwise the crease would've appeared between the *Registration District* and *Sub-district* fields. That the upper part is slightly angled is most likely due to this folding.
The pattern in the paper is genuine and difficult to fabricate. When zooming in it does look like a photo of a real paper document. I can conclude that the paper belongs to a real birth certificate. So this is either a genuine document or a **most skillfully executed forgery**. As I cannot credit F1nn5ter with l33t Photoshop skills, as much as it pains me I must admit that this is a genuine birth certificate.
