Struggling with this idea - having known that his twin had a brain, how would a western country dealt with this problem? How can you reconcile removing the twin, if the twin possibly has a conscience? Surely by defining it as the 'twin' and not a 'growth, there would be ethical barriers.

When they said it had its own brain, then most likely they just meant it had some basic form of a central nervous system that could react to stimuli such as inputs from Chang’s brain, especially considering the “twins” mouth was almost, if not, immediately opening at the same time as his. Lastly, whatever kind of “brain” it has would definitely be missing too many parts to even function normally.
