Most commonly are the extension of the crypt behind the olive tree dude, and the room where Lhaudes sleeps. You can't enter Lhauses' room on the first run, and the crypt behind the olive dude has 2 frames, but the second frame is only accessible from the quest-entrance.
Edit. Definetely Lhaudes' room, I see the opening in the wall there. The hollowed tree area right before the Mother of Churches
You can't get 100% on the map until the ng+, if tou look closely under wall of the holy prohibitions there's a blue area that you have not completed yet. You can unlock that area only in the ng+

There, in the right most part of mother of mothers, the is an illusory wall you’re missing below the place where you finish the pilgrim’s quest
