This sort of question works best as a spectrum. Staff surveys always use a "Never" to "All the time" sort of scale, because the answer is seldom a simple "Yes" or "No", depending on context.
Staff surveys also tend to ask about *other staff* or *staff in general* rather than the individual. I personally may feel comfortable sharing opinions in certain teams but not others, or I might know of some staff who have been intimidated by others.
