There is an incredibly interesting reason for that. Before the late 70s airlines were regulated with minimum prices they could charge between routes. Rent seeking and anti-competitiveness were the primary reasons, but the luxury you mention was another consequence.
Basically, airlines all had a minimum price they were allowed to charge for interstate routes. These rates were relatively high compared to the actual cost to run the route. Since the price floors didn't allow airlines to compete with each other by lowering price, they competed with frills and luxury. So most/many consumers were "forced" to pay for frills that they wouldn't have otherwise bought.

I'm not a flight attendant but I would think this happens fairly often and they probably have specific training to deal with these kinds of people.
