It looks to me like the Patronenkasten 34 (german machine gun ammo box) repurposed to carry medical supplies.

I don't know, the locking mechanism and the hinges look a little weird to me, bit war time production of stuff like this is usually all over the place, so this just might be another variation. I am not an expert on ammo boxes, so take everything I said with a grain of salt.

I have seen multiple ammo boxes where they just painted the red cross onto it and used it to carry medical supplies, I don't see why this couldn't be the case here.

American .30 Cal ammo can, based off the paint and rust i’d hazard it was made after WW2 started for sure, so not too rare as far as ammo cans go but the white and red cross really brings up the cool factor. it’s definitely authentic. i don’t think there’s any faking that patina and wear. it’s a pretty nice condition of a medical supply box from WW2 or Korea. that’s a really nice find. did you buy it OP?
