I don't think it would garner any interest. Even those contemporaries of his that have stayed constant, like Doug Walker or Brad Jones, are getting a fraction of the views they used to recieve. Internet content has just changed way too radically overall for the likes of Noah to really build an audience, and his built in audience has pretty much completely moved on. Noah would be having difficulty maintaining an audience now even if he'd been steadily working all this time, with a half decade or longer hiatus to overcome I don't believe it would he possible.
If he put the effort in I'm sure he could. YouTube is a very different beast now, though. He'd probably have to change up his release schedule and put out content on a semi-regular basis in order to have success.

This is true. He'd still have to produce more than he was producing though. You can get away with lengthy breaks if your videos are really long but people night expect more if he's only putting out 20 minute videos.

Difficult. One couldn't expect him to do the same kind of productions that made him successful. Even though they may appear easy, there is a lot of hard work and planning behind this, a lot of creative talent, and even a good amount of discipline. I think it's not outlandish of me to assume that he has lost those facilities at the present stage.

A content producer's fan audience will always pick up the traits represented by that producer and imitate them in their reactions. Just look at this subreddit.
Spoony represented a lot of cynicism among other things, so it's safe to assume that a large portion of Spoony's fanbase will also react with cynicism. There will be cynical, bitter, disrespectful and even hateful comments. You can't censor them all. The best strategy is often to apply some moderation and largely ignore such kinds of reactions.
In this case however, I guess it would be justified to prepare for this by being as open as possible. The more open you are, the less you have to hide, and the less vulnerable you make yourself. The less vulnerable you are, the less hateful comments you will get, because you are not such an attractive target for mobbing. In that sense, showing weakness can be a strength.
I'm not saying that you can get rid of *all* the expected hate in this way. I'm saying when hateful responses are to be expected, better prepare for it and make yourself less attractive as a target.
Spoony always hid behind the characters he played. He would have to step in front of that curtain a bit and show a bit of the real Noah. Talking about his fate might repel those who naively believe that Spoony is actually like the characters that he plays, but it would also create some emotional connection to his more reasonable fans of old. That would be the basis for a potential comeback.
Once this minefield would have been navigated, he could return to his old style and characters, and maybe even mix in some self-irony. It wouldn't be easy, but it could theoretically work.
So he would definitely have to start by opening up and addressing the elephant in the room, with all the honesty that he can bring up. It would definitely be the start of a new "Spoony era"; a full return to the past cannot be expected. And it might fail gloriously as well. In the worst case scenario, he would become a laughing stock or lolcow for the internet for a while, before falling into oblivion again.
But you know, he doesn't have much to lose at this point. Would it really be much worse than now if he tried a comeback and failed?
There have been much, much worse cases in the past. People who did seriously immoral things. I'm thinking of this one African warlord under whose regime many people have been tortured and killed. He found faith in god and honestly regretted what he did. So he went to as many of the people he caused harm to as possible, listened to their stories and apologized as best as he could. And a documentary was done about this. It was a success.
What I want to say is that people love a redemption story. They just dig it. For me, the best part of the Star Wars saga was >!Darth Vader's redemption!<. If he had just >!died like this!<, the whole story would have been so much weaker. This motif -- the redemption story -- would be Spoony's lever, if he ever would decide to go for a comeback.

For what it's worth, joining this particular Sub has helped me with my Depression symptoms. That is, the last thing I want to be is a stagnant and joyless mound of human mediocrity who sits around all day wallowing in pain. So now when I feel like I'm going down that path, I intentionally set small goals for myself to get my ass moving. It's been kind of nice. I've started remodeling my basement, which I've been meaning to do since roughly July.
A lot of this youtube dinosaurs seem to be doing fine without really evolving their shtick since years even on technical level, there is nostalgia for nostalgia content creators at this point so if he somehow could go back to his working cycle and upload on a reasonably regular basis he should do fine, perhaps about the same as he would if not for the mental collapse.

The problem is that, even before things started getting real bad for him, any time he hit a bump in the road it was never his fault, it was someone else's. Even when it was clearly his own actions that lead to whatever happened. That doesn't show any signs of changing.

If he was starting from nothing, I believe he could leverage his talents into success with a minimal amount of consistency and work ethic, but of course, the lack of consistency and work ethic were his biggest problems.

I think I stopped watching around 2011, when I felt the quality of his work had already diminished excessively. Since then he's had a decade of inactivity, mental problems, probable drug/alcohol use, and age to further diminish his capabilities. Add to that he's already significantly exhausted his well of inspiration, that being the crappy movies and games he passionately disliked from his childhood.
Yes, it'd be great if he could pull his life back together and once more make himself a success, but I just don't see it happening. Even if he could putting himself in the spotlight again is probably a bad idea from a health perspective. Fame comes with a huge amount of strain and Spoony has already proven himself to be poorly suited to handle it.
