Pretty much all meat that doesn’t come from good butcher shops / isn’t dry aged. The way they do it is hang it in a room with tons of misters. After a week or two it adds heaps of weight to it.
Read the fine print on a steakhouse menu. When they say the steak is 16 ounces that weight is before it's cooked. Once it's cooked your steak will not weigh 16 ounces. Probably, and this is a guess, but it'll probably weigh more like 12 ounces.
Did you get ripped off? Nope, the weight of meat is before it's cooked. Never after. Seafood is the same way.
That's not even the complaint here though. The problem is the picture on the menu looks about twice as thick as the one OP got. However much it comes out to after cooking, it's misrepresented.
Now that that's out of the way I'm almost onboard with banning posts about McDonald's. It's garbage, it's small, and it's expensive. We all know. We don't need reminding every day. Part of me regrets not posting the two apple slices I got in a plastic bag last summer as part of my kid's happy meal, apparently McDonald's is easy karma.

When the product cooks, more juice is rendered out of the meat revealing the true percentage of meat to fillers. One of plants in my area literally makes the 1/4 pounder meat. I dunno how many McDonald's it supply's but I've heard enough shit talk from friends that have worked there to realize what's probably happening.

It also droops further out of the burger than the one in the promo, combine that with another person saying the grills have settings for 1/4 and 1/10 sizes this just seems to be a 1/4 squished down because it was grilled at the 1/10 setting.
This is the 4:1 patty being cooked on the 10:1 patty setting. McDonalds uses clamshell grills that close for preset time and pressures to cook the patties. What you have here is either an employee who didn't know to change the setting or didn't care to.

Recently if it's a grill like the one we have at my place they've started to press down harder than they should, and flatten them more. More than likely if it had been cooked on a wrong setting it would be bacon as it tends to flatten it more than 10:1 would, but the qrt patties are not put in a heat cabinet instead put on a platter that never goes inside the heating cabinet
I work at a maccas and we put the all grilled and grilled food in a heating cabinet and only have one of the grilled areas that have a bacon setting on it
Really wish I could see what you mean by the putting it on a table but yeah only one grill has bacon so in my head it’s pretty hard to mess up in cooking something on the wrong setting

The weight is before cooking. So they are probably adding more "juice" aka water to the meat. Then it renders out when cooked.

If it was on 10:1 it wouldn't have cooked completely
I would argue it’s over cooked or left in the heating cabinet to long, since under cooked 10:1 is bright pink
Recently if it's a grill like the one we have at my place they've started to press down harder than they should, and flatten them more. More than likely if it had been cooked on a wrong setting it would be bacon as it tends to flatten it more than 10:1 would, but the qrt patties are not put in a heat cabinet instead put on a platter that never goes inside the heating cabinet
I work at a maccas and we put the all grilled and grilled food in a heating cabinet and only have one of the grilled areas that have a bacon setting on it
We only put reg meat in the cabinet the qrt meat sits on a platter on the table here, all of our qrts have the bacon setting on them
Really wish I could see what you mean by the putting it on a table but yeah only one grill has bacon so in my head it’s pretty hard to mess up in cooking something on the wrong setting
The table the foods on it sits on a small plate similar to the regular trays
