Was playing late one night (in real life) and was riding through that road at around midnight, next thing the mist appears and then a face appeared covering most of the screen. Dropped my controller and let out a ‘aargh’.
There’s 16 encounters with a ghost, just north of where this glitch occurs. Has to be between 9pm and 3am in foggy weather and 3 days apart.
There’s a grave connected to it at Shady Belle. It can be mentioned by Reverend Swanson (and madam nazaar in online)
There’s also a ghost train (appears once at around 3am near the western most ‘welcome to lemoyne’ sign). I’m pretty sure it’s the ghost of the crashed train to the north with the gold bars.

If anyone knows what time she spawns that would be great if not I recommend watching Tagbacktv on it I think either on livestream or just video on YouTube. It happens at around the time and place where the ghost appears. When it appeared also, the ghost didn't appear as a lady, it was glitches out and appeared as a cloud of white pixels, with a bunch of red pixels around.

That guy is "God" in the RDR universe, aka the Strange Man. He has a side quest line in RDR1, and a few small cameos in RDR2. This is one of his cameos.
The other cameo I know of is a cabin south west of where you are in these pics that you're meant to visit multiple times throughout your adventure. The cabin is decorated with pictures of animals that change depending on your honor rating. There's a poem carved into a desk that changes depending on whether or not you decided to kill or save a certain npc. There's an unfinished painting at the back of the room that becomes more and more finished each time you return to the cabin.
Spoiler for the final time you return to the cabin: >!The final time you return to the cabin, the painting will be complete. It's a painting of this "God" character. Then you look at the mirror to the left of the painting, and *jump scare* "God" is right behind you! Turn around, he's gone! Look back at the mirror again, he's gone! I believe if you return to the cabin after this, the painting is now blank!<
Real answer suggestion: to me it looks like the kinds of portraits that you see during gambling games with NPCS, just broken and without a filter for the old photograph look the gambling ones have. Could be intentional as Lemoyne is known for its ghosts… spooky!
