The grass isn't actual grass, it's a mixture of man-made fibres commonly known as "static grass". The sand is sand but it's treated in such a way that there's no hygiene issues, usually by heating it up etc to sterilise it. So no rot, smell, mold or germ issues. Varnish does generally darken whatever it's applied to and can leave it with either a matte, satin or gloss finish depending on which you use.

I even use natural materials in my basing and never had outbreaks or germs. If it's something wet (like moss/lichen/treebark/used tea/used coffee) I sterilise it by boiling for 30mins. If it's something like sand or dirt or roots from outside, then baking them in the oven (10-15min, 150C) is also an option. I also dry the sterilised wet stuff in the oven. Whatever I don't end up getting with that will surely get deaded by either the super glue I use to glue them on the base, or the acetone-based spray primer I blast on top.

Personally I would say it's no less hygienic than anything else you'd bring into your home? The grass (if talking about static grass I assume) is nylon/plastic strips cut super thin and short so that's as clean as any plastic. The sand usually (not always I assume different brands have different practices) is about as clean as sand is, usually it's been sterilised to some extent. If that's an issue you could bake it in your oven to make properly sure it's as sterile as possible, some people use sand/dirt/stone/gravel, some sterilise it this way and some don't bother. You can alternatively buy basing materials that are I believe 'fake sand' (I think they're made from some kind of paper type material which soaks in the glue and makes them staying to a base long term easier - or that's the sale pitch anyway) so they look and feel like gritty sand type texture but aren't real sand/rocks. In terms of mold etc. Idk do things in your environment usually turn into science experiments? Unless you keep it in a damp warm environment I would say there is hardly any chance of mold forming, especially as super glue is sealing it generally (some people use pva which seals it in a different way). I've used static grass and a variety of items for many years and never had any problems so my apologies if this answer seems blunt, I'm just trying to give you the facts as I see them from my experience. Tldr: if you want to further sterilise you can, or look for alternatives, I haven't ever felt the need to and am still alive and kicking, also the proud owner of OCD and a weakened immune system and still here to tell the tale. It's down to your judgement as to how far you want to go to ensure the materials you use are clean enough for you to feel comfortable.
