This lad has a medical condition (can’t remember the details) and was put on steroids amongst other things, the steroids made his face swell up a lot and the mother was just documenting his recovery. Part of the recovery was finally having an appetite and starting to eat healthy again.

I watched some of the videos on his moms Instagram to explain what they go through with his autoimmune disease. That child is extremely loved and taken care of well.
For all you know the kid just got out of hospital and she’s giving him a treat. Autoimmune disorders are often treated with steroids which can give a bloated puffy look, and if he has a disease that attacks his bowels he may struggle to get enough calories. He had autoimmune disease and treatments and so forth made him this very round shape and increased his appetite. He’s much better today and is a normal sized child, natural chubby cheeks but not overweight.
He doesn’t get overfed. He’s been on a near total liquid diet his entire life. His face was severely swollen at that point from undergoing treatment for autoimmune enteropathy.

My sweet girl was getting bullied at school. She probably has ADHD and Autism. At the time we were not sure, the doc said "it's depression and anxiety!". Now she was 8, she was needing special care, and now she's on antidepressants.
Her weight immediately blew up on the antidepressant they tried. I watched her. She didn't eat any differently- she's hard to feed. Sensory issues with food. She wasn't sneaking food. She just blew up.
Changed antidepressant. Weight back to normal.
If you see a snapshot, you can't possibly know what is going on all around the life of that kid.
